State the needs and specific intentions for funds requested.

If not already in our files, submit your organization’s non-discriminatory policies, your AWSNA/WECAN status and proof of 501(c)(3).

Please be sure to include:

A report of the use of our grant last year, if applicable.

Current year’s budget, income, and expenses for year to date.

Income and expense statement and balance sheet of previous fiscal year. Annual audit may be submitted.

A proposed budget for the specific project, showing income and expenses, if applicable.

If funding is for a new program, explain how the program would be funded in successive years.

A list of other financial resources being contacted for this expense.

The date funding is needed if outside of normal school year.
